FormLizard helps its users create attractive, functional forms. The user can begin by using their current form documents. After applying tags to the document using their word processor, the user can save the item as an RTF file and upload it to FormLizard. The application automatically reads the applied tags and generates fields. Rather than create a literal online form, FormLizard instead translates the information into an online interview. Instead of plain fields that need filled out, the application turns each into a question. The user answers the question and FormLizard automatically translates it into a completed document. The application is also designed for convenience, with a fill out pause function that allows the user to resume progress on their form later. Users can also re-use a previous interview by making any necessary changes. FormLizard also provides an interview sharing feature that makes it easy for users to collaborate on forms. FormLizard supports Google Docs, Box.net and RightSignature integration. Users are also protected by 256 bit AES encryption for security sensitive forms. Additional features include the user’s branding, help buttons and conditional fields.

FormLizard keeps things simple, right down to the layout and design. The user can easily navigate through existing fields or create new ones. After completion of the form, the user is provided with a document that is filled out neatly and completely. The logo upload option also allows users to create a form that matches their business or organization.
A new user can create a FormLizard account by clicking the orange “Sign Me Up!” button found along the left hand side of the homepage. The following page asks the user to select their preferred membership option. After clicking the corresponding orange button, the user is taken to the registration form (which looks much like the ‘interviews’ the application generates). The form asks the user for their email address, password, first and last name, company name, intended use, custom FormLizard URL and credit card information.
FormLizard provides its users with five membership options to choose from. The first is a free Chameleon membership that allows one form, unlimited form completions, unlimited users, Word and PDF download capabilities, Excel data export features, e-submit capabilities, 256 bit SSL security, an individual secure database and API access. The Iguana account costs around $15 per month and includes up to five forms, public access, conditional fields, personalization options and the ability to send completed documents to third party integration partners. The Monitor Lizard account costs around $40 per month and allows up to 15 forms and all the above mentioned features. A Gila Monster account is available for around $60 per month which upgrades to 25 forms while the high end Komodo Dragon account allows up to 50 forms and costs around $100 per month.
